It is not possible to replace the blank page that you write on in Nebo with a form that you can then fill in or that you can write over. To get around this you can create a template that uses standard Nebo formatting and then "fill it in" separately when you are ready to do so. You can also keep a copy of this template as a separate .nebo file that can then be used whenever it is needed.
